Our Origin: Born from a Problem, Built for a Purpose
Medical Exercise Training was born from necessity—not marketing.
In 1994, as insurance carriers slashed physical therapy reimbursement, we saw physical therapy patients discharged before they were fully functional. The healthcare system was efficient but incomplete. Our clinics could not extend care, and the fitness industry was unprepared to continue it safely.
So, we built a bridge.
We designed a structured, medically guided approach to exercise that respected both science and scope of practice. The first Medical Exercise Specialist Workshop was held in July 1994. Thirty-two years later, the mission stands stronger than ever: to equip professionals to manage medical conditions through exercise—safely, effectively, and professionally.
Our Identity: More Than a Certification

The Medical Exercise Specialist (MES) represents competence, integrity, and accountability in managing clients with medical conditions using exercise—not treatment, not therapy.
We dose exercise with precision, document outcomes with discipline, and communicate with the medical community in their language—objective, measurable, and reproducible.
Our name—Medical Exercise Specialist—was chosen in collaboration with physicians, surgeons, and cardiologists because it conveys exactly what we do: apply exercise as a form of medical management, measurable in dosage and effect. It is recognized and respected worldwide because it was built from collaboration, not imitation.

Medical Exercise Specialists operate within a defined scope of practice:
We manage clients who have completed medical care and are ready to restore function through structured, safe, and effective exercise programming after therapy is over.
We communicate, document, and report—ensuring every program reflects professionalism and accountability.
Beginning in 2026, the Medical Exercise Training Institute (METI) will enforce stronger professional standards across all certifications, programs, and network participants. Every MedExPRO will be expected to demonstrate:
